Accident/Incident Procedures

Documentation helps prevent the loss of important facts relating to an incident, in case the event later evolves into something larger than first expected.  If you observe an accident or a situation where there's potential for injury or damage, take prudent steps to protect people and property, investigate, and note in writing all facts at the time.  If possible, include the names, addresses and phone numbers of all witnesses. 

Reporting an Incident

All incidents should be thoroughly investigated and an Incident Report forwarded immediately to the Safety and Security or Risk Management Department.  Incidents that involve an injury should be referred to the school nurse, who will complete and forward an Injury Report to the appropriate department.

Serious injuries or significant property damage should be reported immediately to the Risk Management Department at ccash@nsd.org or 425-408-7630.

When a District Vehicle is Involved

Accidents involving district vehicles should be immediately reported to the Transportation Deptartment at 425-408-7914, 425-408-7916 or 425-408-7900.

Incident Report Required

There are incidents that require mandatory completion of the Incident Report Form and must be sent to the Safety and Security Department for review and record retention. The following are listed on the Incident Report form but are not meant to be exhaustive:

  • Arson
  • Assault
  • Drugs/ Alcochol
  • Fire Alarm
  • Police related incidents
  • Theft
  • Threats
  • Trespass
  • Vandalism
  • Weapons
  • Building damage

The Safety and Security Department should be contacted for guidance when incidents occur that could potentially become a risk and/or safety issue.
